Intra gastric balloon is an adjuvant to bariatric surgery when BMI is ≥ 30 kg/m2, not a replacement.
It is indicated for temporary weight loss in those who are unfit for bariatric surgery. It is a bridging procedure before surgery to induce weight loss, so as to make these individuals fit for surgery. Intra gastric balloon is introduced by endoscopic method without the need for any cuts on the abdomen. It is simple, very safe and doesn’t require admission.
Once introduced, it occupies part of the stomach and createssense of fullness. It reduces the amount of food taken each time. So intra gastric balloon acts mainly by mechanical restriction. Its effect, on hormones controlling body fat stores,is minimal. So body fat set point is not altered much. Older balloons such as Bioenteric Intragastric Balloon (BIB) are kept only for 6 months. Newer balloons such as Spatz 3 Adjustable Balloon, can be left in place for one year. Advantage of Spatz Balloon is that it can be adjusted, if weight loss is not adequate after 3 to 4 months. After one year it has to be removed. After balloon removal, if bariatric surgery is not performed, all the lost weight will be regained in most of the individuals, because of the effect of hormones controlling fat set point. So weight loss after gastric balloon is temporary when it is performed in individuals with BMI≥ 30 kg/m2.
But it is an excellent choice for weight loss for individuals <30 kg/m2. It is considered as an adjuvant to healthy diet, exercise and behavioural modification in these individuals. In these, since weight burden is not much, fat set point can come down to some extent by healthy diet and exercise. And intra gastric balloon helps to increase weight loss. After balloon removal, these individuals should stick on to healthy diet and exercise in order to keep fat set point nearer to normal limits.
