Mrs. Sridevi underwent Laparoscopic sleeve gastrectomy elsewhere in 2018. She reduced weight from 113 kg to 92 kg in 1 year but failed to lose weight further with lifestyle modifications. She was still 35 kg overweight. She also developed infra umbilical hernia meanwhile.
I performed revision bariatric surgery [Laparoscopic loop duodenal switch (LDS), also known as single anastomosis duodenoileal bypass (SADI)] in June 2019 on her along with laparoscopic mesh repair for hernia.
She lost additional 26 kg after revision bariatric surgery. She got rid of high BP, diabetes and knee joint pains after revision surgery. She is active, fit and healthy now.
Even though weight loss and diabetes remission are long lasting after popular bariatric surgeries like sleeve gastrectomy and gastric bypass, some individuals regain lost weight and get their diabetes back, mainly due to not sticking on to lifestyle modifications.
If lifestyle modifications fail to induce weight loss or diabetes remission again, then revision bariatric surgeries are needed in the form of adding ileal diversion.
Sleeve gastrectomy with loop duodenal switch (SLDS, Single anastomosis duodenoileal bypass with sleeve – SADI-S) is one of the most powerful bariatric and metabolic surgeries, with significantly less malnutrition compared to biliopancreatic diversion with duodenal switch (BPD-DS). Hormonal changes responsible for weight loss and diabetes remission are very powerful after SLDS (SADI-S).
If anyone regains weight or get diabetes back after sleeve gastrectomy or gastric bypass, conversion to loop duodenal switch (LDS) results in powerful hormonal changes required to reinduce weight loss and diabetes remission.
